Community Health Intervention Proposal
Purpose
This assignment aims to engage students in the creation of an innovative community health intervention and the development of a detailed proposal to address a pressing public health issue. Students will analyze health information, perform a community needs assessment, and design evidence-based strategies tailored to the specific needs of a community.
This assignment focuses on leveraging human empathy, ethical reasoning, and contextual understanding—distinctively human skills—to design interventions that prioritize community well-being and address social determinants of health.
Learning Outcomes
- Analyze and interpret health information and data to identify priorities and SMART goals (research analysis)
- Conduct a comprehensive community needs health assessment (community assessment)
- Compare and contrast varied interventions to address a community health issue (community assessment)
- Create an evidence-based recommendation for a community health intervention (intervention design)
- Create a logic model to illustrate the intervention (intervention design)
- Articulate recommendations for planning, implementation, management, and evaluation (proposal)
Instructions
- Identify a Public Health Issue
- Choose a public health issue affecting a specific community. Possible issues include but are not limited to obesity, diabetes, substance abuse, mental health, or infectious diseases.
- Research Analysis
- Conduct thorough research on a chosen public health issue. This initial research will focus on a global scale. Gather information, data, and evidence that will be used as the foundation of the Community Health Intervention Proposal.
- Research the following elements:
- Incidence and prevalence rates
- Morbidity and mortality rates
- Patterns (e.g., age, gender, race, socioeconomic status)
- Long term trends
- Acute changes to long-term trends and patterns (if applicable)
- Economic impact
- Environmental impact
- Political and policy landscape (i.e., existing policies, current regulations, and programs)
- Research the following elements:
- Use at least five credible sources, including but not limited to:
- Public health records
- Academic journals
- Government reports
- Community health surveys
- Community health assessments
- Environmental reports
- Synthesize the research findings
- Interpret the research results within the context of the public health issue. Answer the following questions:
- What are the key findings, patterns, and trends?
- What are the potential causes of the health issue?
- What are the implications for the findings?
- Interpret the research results within the context of the public health issue. Answer the following questions:
- Conduct thorough research on a chosen public health issue. This initial research will focus on a global scale. Gather information, data, and evidence that will be used as the foundation of the Community Health Intervention Proposal.
- Are there varying perspectives on the health issue? If so, identify and explain.
- Community Assessment
- Perform a community assessment to identify the demographics, cultural factors, and existing resources related to the issue.
- Create a community profile.
- Identify the community and why it was chosen.
- Highlight its characteristics, such as its population, demographics, socioeconomic status, and existing cultures
- Examine social determinants of health within the community.
- Housing
- Healthcare
- Education
- Income
- Food access
- Environment (e.g., water, air quality)
- Access to social services (e.g., mental health, recovery)
- Conduct secondary research that is focused on the community and will aid in assessing the potential impacts of future interventions. Research the following:
- Incidence and prevalence rates
- Morbidity and mortaility rates
- Patterns (e.g., age, gender, race, socioeconomic status)
- Long term trends
- Local or regional policies, regulations and/or programs
- Local community based resources
- Existing intervention programs and their effectiveness
- Synthesize findings
- Interpret the results of the research within the context of the community health issue. Provide evidence-based answers.
- Clearly define the health issue(s) for the community
- What are the key findings and takeaways?
- How are the social determinants of health impacting the community health issue?
- Are there existing community-level interventions? If so, are they effective or ineffective?
- Are there any disparities in how the health issue affects people from diverse demographic groups? If so, how and why.
- What do you perceive to be the highest priority in impact on the community? Why?
- Clearly define the health issue(s) for the community
- Community Assessment
- Intervention Design
- An effective intervention design will:
- Build upon or use evidence-based interventions with proven effectiveness
- Adapt to the target community by considering regional contexts
- Use and/or build upon existing community resources and programs
- Is accessible and considers potential barriers to access
- Is sustainable (i.e., implementation, funding, monitoring, maintenance, community engagement)
- Considers ethical issues
- An intervention design will include the following:
- target community and population
- Problem statement
- Specific, Measurable, Achievable, Relevant, Time-bound (SMART) goals
- rationale for the intervention supported by research, health data, and community assessment findings
- focused strategy to addresses the identified social determinants of health
- defined outcomes
- Create a simple logic model that illustrates the relationships between the resources, activities, outputs, and outcomes of the intervention. It.
- Outline the following elements with brief descriptions:
- Inputs (e.g., resources)
- Activities (e.g., workshops)
- Outputs (e.g., products)
- Anticipated outcomes for the short, intermediate, and long term
- External factors that will impact each element of the program (e.g., public policy)
- Outline the following elements with brief descriptions:
- An effective intervention design will:
- Community Health Intervention Proposal
- Create a detailed proposal for a community health intervention that addresses the identified issue.
- The proposal will:
- Articulate recommendations for planning, implementation, management, and evaluation of the effectiveness of the intervention.
- Group Presentation
- Groups will take on the role of a consulting firm presenting to a regional public health authority board. The board will determine whether to move forward with a collaboration on a project that would involve some or all of your proposed intervention. The representatives from your team will be charged with clearly communicating the problem, research, data, current state of health, impacts on the quality of life of community members, alternative solutions, and the proposed intervention. The presentation should include recommendations for planning, implementation, management, and evaluation of the effectiveness of the intervention.
